The app is not showing up after updating my theme version.
I just updated my theme from 10.0 to 11.0 (Same theme: Testament), and the app has completely disappeared.

You may need to re-add the review template. First go to the Template tab and modify the review template and save it. That will re-create the code for your theme. Then you can follow the instructions to re-add.
